A cheap handshake test predicts more than most lab panels

Squeeze a simple spring-loaded gauge once and researchers can estimate something your bloodwork often can't reach.

A hand dynamometer is about as unglamorous as medical equipment gets: a metal spring you squeeze once, producing a number in kilograms. For decades it sat in physiology labs as a minor fitness measure. Then the PURE study — a long-running project tracking roughly 140,000 adults across seventeen countries — started publishing results that made grip strength look less like a fitness footnote and more like a signal worth watching.

Across that data, weaker grip strength tracked with higher rates of death from any cause over the following years, and the association held even after researchers adjusted for age, smoking, and diagnosed disease. The size of the association surprised people who ran the numbers: in some analyses it out-predicted systolic blood pressure, a measurement doctors have trusted for a century.

Nobody thinks squeezing a gauge harder makes you healthier — grip strength is a marker, not a mechanism. The leading explanation is that it's a rough proxy for overall muscle mass and the systems that maintain it, which quietly decline for reasons unrelated to your hand at all. Weak grip doesn't cause anything. It's a small, easily measured window onto a much larger and slower story happening throughout the body.

The instrument stayed simple because the thing it's catching didn't need to be complicated to matter.
